Pay £27- £33 an hour

We are seeking a practice pharmacist with previous experience of working in primary care to develop and manage medicines optimisation services within our general practices.

  • Medication Reviews
  • Discharge Reviews
  • Minor Ailment clinics
  • Manage repeat prescription authorisations, and reauthorisation
  • Manage medicines at transfer of care
  • Diabetes/Hypertension/respiratory speciality preferred but not essential
  • Potential to work explore work in care home and or intermediate care
  • Participate in Quality improvement, clinical audit and QOF

CANDIDATE REQUIREMENTS

  • Submit Your CV and Compliance Form
  • 5 years post registration
  • Minimum 12 months experience in working in a General practice
  • post-graduate diploma or higher degree in clinical pharmacy
  • An independent prescriber
  • Minimum of 12 months UK, NHS experience working in either community/hospital/primary care
  • Know how to support QI for the practice
  • Undertake practice performance QoF, prescribing LES, safety alerts, formulary, waste
  • Identification such as valid passport or driving licence
  • Proof of address x 2 Proof that you have a valid visa to work in the UK
  • DBS Certificate
  • NI number
  • Last five years of employment history Professional references x 2
  • Proof of immunisation (Hep b, Varicella, Measles, Rubella & TB)
  • Up to date training certificates Professional qualifications Proof of professional registration (NMC/GPHC)
